In order to receive your payment, all you need to do is submit a Form W-4 and a Form W-9 to Microchip at the link HERE and below. While you must submit the forms no later than 45 days after the Effective Date to be paid, you are encouraged to submit them promptly to facilitate prompt payment of your share. You do not need to wait until after final approval of the Settlement to submit your forms—you can submit them as soon as you decide that you want to participate in the Settlement.
